American Mattress Manufacturing Recalls Renovated Mattresses Due to Violation of Federal Flammability Standard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Mattresses and mattresses with foundations

Units: About 1,500

Manufacturer: American Mattress Manufacturing, of Atlanta, Ga.

Hazard: The mattresses fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.

Incidents/Injuries: None reported

Description: This recall involves American Mattress Manufacturing’s renovated (rebuilt) twin, full, queen and king-size mattresses, and mattresses with built-in Bunkie boards. The used mattresses were stripped to the springs and rebuilt by American Mattress Manufacturing in a variety of fabrics and colors. The recalled mattresses have a yellow tag with “American Mattress Mfg. Co., 1899 Metropolitan Parkway, Atlanta, GA 30315.” Rebuilt mattresses with a federal tag, indicating they meet the federal mattress flammability standard, are not included in this recall.

Enesco Recalls Shelly's Diner Collectible Due to Fire Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Shelly's Diner® Collectible Ceramic

Units: About 1,600

Distributor: Enesco, LLC, of Itasca, Ill.

Hazard: The diner's power adapter can overheat and melt the adapter's plastic housing, posing a fire hazard.

Incidents/Injuries: Enesco has received two reports of power adapters partially melting due to overheating. No injuries or property damage were reported.

Description: The recalled product is part of the Department 56 Original Snow Village collection of lighted buildings and comes with a traditional power cord that lights the interior and a separate power adapter that lights the exterior. The collectible is a ceramic building shaped like a diner and is approximately 10 1/4 inches long by 6 1/4 inches high and 4 1/2 inches deep. This recall includes diners with date codes 03 12 JP01 or 05 12 JP01 stamped on the bottom of the diner.

Sold at: Holiday stores and gift shops nationwide and online at amazon.com from July 2012 through September 2012 for about $120.

Catalina Outdoor Fireplace Recalled by Christmas Tree Shops Due to Burn and Laceration Hazard

Importer: Nantucket Distributing Co., Inc. of Middleboro, Mass.

Hazard: The glass components of the Catalina Outdoor Fireplace can break when a fire is lit posing a burn and laceration hazard.

Incidents/Injuries: Christmas Tree Shops received three reports of the glass component of the outdoor fireplace breaking. No injuries have been reported.

Description: The Catalina Outdoor Fireplace is free standing and constructed of glass and black metal. The 21 1/4 in. square base sits on short legs. Black corner pieces hold the glass sides with one hinged side to add logs. It has a square, vented top lid and stands about 30" tall. The model number CT840BL appears on the outside of the packaging and on the instruction manual.

Sold at: Christmas Tree Shops retail locations from September 2011 to October 2012 for about $130.

Description: This recall involves seven brands of sweat and threaded, low lead ball valves used in flammable gas piping systems used in commercial or residential settings. They are brass shut-off valves assembled with pipe lines of 3/4 inch and 1 inch and have a date code 1103 through 1112, in YYMM format, where YY is the year and MM is the month. The date code is located under the valve handle. The pipe size is marked on the body of the valve. Fu San's logo and the "O" marking designated for low lead valves are marked on the valves' neck under where the handle assembles. The word "Taiwan" appears on the handle as a country of origin marking. The valves were sold under the following brand names and model numbers:

Remedy: Consumers should turn off the gas supply until a replacement gas valve has been professionally installed. Consumers should contact Danville Sales which, on behalf of Fu San Machinery, will provide compensation of $300 to reimburse customers for costs incurred to remove and return the valve and for purchasing and installing a replacement valve. The affected valve should be returned to the Danville Sales Office for Fu San Machinery with a photo depicting the valve in the gas line before removal and showing the date code on the valve, along with the address and details of the location where the valve was installed. The requested information, the recalled valve, and the consumer name and address should be sent to the address in the Consumer Contact paragraph. Upon receipt, Fu San will reimburse customers in the amount of $300.

Description: This recall involves four models of Jotul and Scan brand gas fireplace inserts. These inserts fit into existing vented fireplaces providing a variety of fireplace effects fueled by natural gas or liquid propane. The fireplace inserts are cast iron or steel and have a glass front, ceramic fiber logs, a gas burner and an electrical cord. They measure about 17 inches by 32 inches. The model and serial numbers are located on a metal rating plate inside the bottom front panel of the fireplace insert.

The drill's model number is located on a yellow label on the left side of the drill. "Made in China" appears in black and red lettering on a yellow warning sticker located on the right side of the unit.

Direct Selling Companies Donate More Than $16.6 Million to TODAY Show Toy and Gift Drive

Washington, D.C. (Nov. 19) – The Direct Selling Association helped kick off the 19th Annual TODAY Show Holiday Toy and Gift Drive this morning by presenting the direct selling industry’s donation live in Rockefeller Plaza.

Nineteen direct selling companies donated a total of more than $16.6 million in products and cash to the Toy Drive. Of the 19 participating companies, at least 10 will be featured in their own spots on the TODAY Show during the holiday season. In its nine years of participation, DSA member companies have donated more than $100 million in products, services and cash to the Toy Drive.

Participating companies donated items from books, stuffed animals and fashion accessories to clothing and toiletries for children and families across the country.

"The TODAY Show Holiday Gift Drive is just one example each year of how direct selling serves as a force for good in our communities, across the country and throughout the world," said Amy Robinson, DSA’s Chief Marketing Officer. "Every day, direct sellers help others achieve their goals through the products and opportunities they offer, but their involvement in charitable and philanthropic activities reinforces the positive impact direct selling has on millions of lives each year."

The TODAY Show Toy Drive is a project of the Today Show Charitable Foundation, Inc., a 501(c)3 non-profit organization. House Fire Kills Four in Lake City

A family in Lake City lost four people in an overnight fire: 22 year-old Tyra Pierson and her 10 month-old son Xavier, as well as Tyra's two young siblings that she was babysitting: eight year-old Madison, and three year-old Wyatt.

 

Sometime before 1:40 a.m. Tuesday morning, the house at Illinois street in Lake City went up in flames.

 

"We knew we had a pretty good size problem to start with," said Lake City Fire Chief Mel Alcox.

Four people were trapped inside, two upstairs and two downstairs.

"I tried to gain access to the residence but couldn't because of the smoke and heat," said Lake City Police Officer Dane Cook, who first noticed the smoke and flames.

The smoke and heat were so intense that it took firefighters over four hours to put it out.

"Oh, it was ablaze!" said Alcox. "Like I said, within probably five minutes after we got here, maybe even 2 or 3 minutes, I said to myself if there's somebody in there we are going to have a problem saving them."

Family members gathered outside later in the morning...sorting through rubble, trying to pick up the pieces.

The fire marshal says an electrical cord from a hanging lamp got pinched between a hide a bed, causing it to heat up.

"Unfortunately it put an overload on two plug-ins, and that caught the wall on fire," said Alcox.

The chief says in his 35 years in Lake City, he can't remember a fire that killed multiple people.

The chief also said there may have been another problem:

"We never heard any smoke alarms," said Alcox. "We didn't ask the owner if there was any, but usually you hear them. We would have heard them right off the bat when we got here, and there was no smoke alarm making any noise."

According to the Iowa State Fire Marshal's office, 28 people have died in fires so far this year.

Incidents/Injuries: Atomi has received 13 reports of the chargers overheating, smoking and acrid smells. No injuries have been reported.

Description: This recall involves Sharper Image USB wall chargers with model numbers “TSI202” and “TSI260.” The wall chargers are used to recharge electronic devices through a USB connection including MP3 players and other devices. Cardboard Boxes Blamed in Maine Fire That Killed 4

ORRINGTON, Maine November 12, 2012 (AP)

A pre-dawn fire that killed a 30-year-old man and his three children and hospitalized the mother was caused by empty cardboard pizza boxes stored too close to a wood-burning stove, Maine fire officials said Monday.

The family likely used the boxes to help start the fire in the stove after they returned to their Orrington home from bowling late Friday night, fire investigators said. A container of lighter fluid found nearby probably accelerated the spread of the fire once the boxes stored outside the stove ignited and family members had gone to bed.

Fire Marshal Joe Thomas called the fire a tragedy. It was the deadliest fire in Maine in 20 years.
"It was purely accidental," Thomas said.

Neighbors reported the fire about 2:30 a.m. Saturday after hearing Christine Johnson, 31, screaming from atop a roof above a breezeway between the house and a garage.

Johnson remained hospitalized for smoke inhalation on Monday. The fire killed her husband, Benjamin Johnson III, along with their children: two boys, 9-year-old Ben and 4-year-old Ryan, and one girl, 8-year-old Leslie.

The children were found in a second-story bedroom, and their father was found at the head of the stairs. They all died from smoke inhalation, according to the medical examiner's office.

The home's furnace was not working, so the residence was heated with the wood stove and a propane heater placed inside a fireplace, officials said.


The home had smoke detectors, but neighbors who rushed to the scene didn't hear them and investigators haven't determined if they were in working order, Thomas said.

The town of Orrington has about 3,900 residents and serves as a bedroom community to Bangor, six miles to the north. The family moved from Bangor and into the rental house about six months ago.
Benjamin Johnson worked at a Wal-Mart and as a part-time dealer at a casino in Bangor, neighbors said. Christine Johnson is an author who worked from home, Department of Public Safety spokesman Steve McCausland said.

The fire was the deadliest in Maine since 1992...

With heating season upon Maine, Thomas said combustible items should be kept at least 3 feet from wood stoves and flammable liquids should never be used to start wood-stove fires.

"The point we're trying to make is this type of thing doesn't have to happen," he said.

Description: This recall involves Master Forge four-burner gas grills with a single-door base. "Master Forge" is written on the grill's hood. The model number GD4825 is located on a label inside the door of the grill's base. "Item 94227" is written on the cover of the instruction manual.

Boy, 5, dies in Edgewood townhouse fire Friday

A 5-year-old boy died in a fire that started in the basement of an Edgewood home Friday evening, state police said.

A joint investigation by the Office of the State Fire Marshal and Maryland State Police got under way immediately at the scene of the one-alarm fatal fire at 1417 Charlestown Drive in Edgewood, the fire marshal's office said late Friday night.

 

The 6:25 p.m. fire was contained to the basement of the two-story wood frame, center unit townhouse by the first arriving firefighters, investigators said.

Approximately 45 firefighters from fire companies in Harford and Baltimore counties controlled the blaze within 25 minutes, the fire marshal's office said in a news release.

The home was occupied at the time by the boy, Cameron Wharton, his father and 2 year-old sister. The father, Harry, escaped with the daughter, Khamyah, but was not able to save his 5-year-old son, the fire marshal's release said.

Investigators have determined the fire originated in the basement of the home. The cause remained under investigation Tuesday.

The children's mother, Latoya Wharton, had left to go to the store with her 15-year-old son. A neighbor reported the fire then called the mother to notify her.

The fire marshal's office said working smoke alarms were operating at the time of the fire.

When you buy a smoke detector, you assume it will sound quickly in a fire, giving you plenty of time to escape. But some experts warn that's not always true. In fact, we found that the most common type of smoke detector — the kind you probably have in your house right now — may not go off in time, even when surrounded by thick, toxic smoke, giving little warning to get your family out.

Amanda Debuty awoke to a house full of smoke, her children trapped upstairs. “As I'm trying to get upstairs, my first thought is the four people that I have upstairs, that they’re not scared, that they’re safe,” she said tearfully.

Tragically, the kids didn't make it. Cause of death: Smoke inhalation. So why didn't they have more warning? After all, Amanda said, the house had working smoke detectors.

“We put fresh batteries in the smoke detectors, we pushed the test button, so I knew they worked,” Amanda said. “And then when it was time, they never went off.”

Amanda said she had the common type of smoke detector, used in 90 percent of homes: inexpensive, easy-to-find alarms that rely on “ionization” technology. They work well to detect fires with fast flames. But experts say some of the most deadly fires are the smoldering, smoky kind that can fill your home with toxic gases while you sleep.

 

Rossen Reports: Arsenic levels in rice trouble experts

In those fires, experts say, ionization alarms don't work well, going off way too late — or not at all. “And that means the individuals could have a fire in their home and never receive a warning,” said Dr. Don Russell, an engineering professor at Texas A&M who’s run hundreds of tests.

Dr. Russell says that while it is “reasonable” for a consumer to assume that a smoke detector will sound when there's smoke, it’s a wrong assumption to make. “It's very scary and that's why people die every year because of this problem.” His findings are a bombshell in the industry — that the most popular smoke detectors may not help you in a fire.

An alarming testWe had Dr. Russell set up a test at the Texas A&M Engineering Extension Service. First he placed three ionization detectors, the kind most of us have, in a room with a couch. Next, firefighters set a slow, smoldering fire, using a soldering iron.

Firefighters say every minutes counts to get your family out, so you want the earliest warning possible. But in our test, the room filled up with smoke and the smoke detectors still hadn't gone off after 30 minutes.

Finally, at 36 minutes, one of the three detectors sounded. Minutes later, the other two went off — just as the couch was about to erupt in flames. “It's way too late, it's too dangerous," Dr. Russell said. "You couldn't get out of that room reliably."
 

Rossen Reports: Hidden stair flaws imperil kids

Remember, this was the type of smoke detector most of us have. But there's another technology out there that experts say gives you better warning in those fires. It's called a photoelectric detector, and even government tests show it goes off much sooner in smoky fires.

Dr. Russell set up another test — this time with a photoelectric next to those three ionization detectors.

Seventeen minutes in, with barely any smoke in the room, the photoelectric sounded the alarm. “Photoelectric is telling us you've got a fire, get up, solve the problem, get out of the house,” Dr. Russell said.

Rossen Reports: Is your child breathing radon at school?

“And what are the ionization detectors telling us?” we asked. “They're asleep.”

Meanwhile, toxic smoke overtook the room. In fact, it took another 21 minutes before any ionization detectors went off.

The seasoned firefighters who observed the test were shocked. “All I could think about is my own family — my own family and my kids trying to get out in that, and if I would've relied on ionization, my family probably wouldn't make it out,” said Houston firefighter Brian Lien. “With the photoelectric they would've had plenty of time to get out.”

While the leading smoke detector companies do make photoelectric alarms, they still sell most of their products without the technology.

“I believe it’s a business decision,” Dr. Russell said, citing the fact that photoelectric alarms are more costly to make than ionization alarms.

Every day Americans experience the horror of fire but most people don't understand fire.

Fire is FAST!
There is little time! In less than 30 seconds a small flame can get completely out of control and turn into a major fire. It only takes minutes for thick black smoke to fill a house or for it to be engulfed in flames. Most deadly fires occur in the home when people are asleep. If you wake up to a fire, you won't have time to grab valuables because fire spreads too quickly and the smoke is too thick. There is only time to escape.

Fire is HOT!
Heat is more threatening than flames. A fire's heat alone can kill. Room temperatures in a fire can be 100 degrees at floor level and rise to 600 degrees at eye level. Inhaling this super-hot air will scorch your lungs. This heat can melt clothes to your skin. In five minutes, a room can get so hot that everything in it ignites at once: this is called flashover.

Fire is DARK!
Fire isn't bright, it's pitch black. Fire starts bright, but quickly produces black smoke and complete darkness. If you wake up to a fire you may be blinded, disoriented and unable to find your way around the home you've lived in for years.

Fire is DEADLY!
Smoke and toxic gases kill more people than flames do. Fire uses up the oxygen you need and produces smoke and poisonous gases that kill. Breathing even small amounts of smoke and toxic gases can make you drowsy, disoriented and short of breath. The odorless, colorless fumes can lull you into a deep sleep before the flames reach your door. You may not wake up in time to escape.
Only when we know the true nature of fire can we prepare our families and ourselves.

Home Fires

• One home structure fire was reported every 85 seconds in 2010.
• Most fatal fires kill one or two people. In 2010, 19 home fires killed five or more people. These 19 fires resulted in 101 deaths.
• In 2010, U.S. fire departments responded to 369,500 home structure fires. These fires caused 13,350 civilian injuries, 2,640 civilian deaths, and $6.9 billion in direct damage.

Escape Planning

• According to an NFPA survey, only one-third of Americans have both developed and practiced a home fire escape plan.
• Almost three-quarters of Americans do have an escape plan; however, less than half actually practiced it.
• One-third of Americans households who made and estimate they thought they would have at least 6 minutes before a fire in their home would become life threatening. The time available is often less. And only 8% said their first thought on hearing a smoke alarm would be to get out!
  Smoke Alarms
• Almost two-thirds (62%) of reported home fire deaths resulted from fires in homes with no smoke alarms or no working smoke alarms.
• Working smoke alarms cut the risk of dying in reported home fires in half.
• In fires considered large enough to activate the smoke alarm, hardwired alarms operated 92% of the time, while battery powered alarms operated only 77% of the time.

Cooking

• Cooking has been the leading cause of reported home fires and home fire injuries since 1990. Unattended cooking was by far the leading cause of these fires; Two-thirds of home cooking fires began with ignition of cooking materials, including food, cooking oil, fat, or grease .
• Cooking caused two of every five (42%) of reported home fires, roughly one of every seven (15% ) home fire deaths, and two of every five (37% ) home fire injuries, and 11% of direct property damage from home fires in 2010.
• Ranges accounted for the 58% of home cooking fire incidents. Ovens accounted for 16%.
• Children under five face a higher risk of non-fire burns associated with cooking than being burned in a cooking fire.
• 90% of burns associated with cooking equipment resulted from contact with hot equipment or some other non-fire source.

Heating

• Heating equipment was the leading cause of reported home fires in the 1980s and has generally ranked second since them. It is the second leading cause of home fire deaths. Fires involving heating equipment peak in December, January and February, as do deaths from these fires.
• The leading factor contributing to heating equipment fires was failure to clean, principally creosote from solid fueled heating equipment, primarily chimneys.
• Portable or fixed space heaters, including wood stoves, were involved in one-third (32%) of home heating fires and four out of five (79%) home heating deaths.
• Half of home heating fire deaths resulted from fires caused by heating equipment too close to things that can burn, such as upholstered furniture, clothing, mattresses or bedding.

Smoking Materials

• In 2010, smoking materials started and estimated 17,500 home structure fires, resulting in 540 deaths, 1,320 injuries and $535 million in direct property damage. Smoking materials are the leading cause of home fire deaths.
• Sleep was a factor in two of every five home smoking material fire deaths. Possible alcohol impairment was reported in one quarter of these deaths.
• In recent years, Canada and the United States have required that all cigarettes sold must be “fire safe,” that is have reduced ignition strength and less likely to start fires.

Electrical

• Half (49%) of home electrical fires involved electrical distribution or lighting equipment. Other leading types of equipment were washer or dryer, fan, portable or stationary space heater, air conditioning equipment, water heater and range.
• In 2010, electrical failures or malfunctions were factors in an estimated 46,500 home structure fires resulting in 420 deaths, 1,520 injuries and $1.5 billion in property damage.

Candles

• On average, there are 35 home candle fires reported per day.
• More than one-third of these fires started in the bedroom.
• More than half of all candle fires start when things that can burn are too close to the candle.
• In 2010, candles caused 3% of home fires, 4% of home fire deaths, 6% of home fire injuries and 5% of direct property damage from home fires.
